Last name, Given Name, Email address, alternate Email address(es), State (initials), Position**,
Special Project Name or County/Town, Date when change occurred.

**Position Acronyms:
SC = State Coordinator CC = County Coordinator
ASC = Assistant State Coordinator ACC = Assistant County Coordinator
LT = Leadership Team
SP = Special Project Coord (or what ever it is), etc.

If there are no changes for a quarterly update, simply enter "no changes" in the appropriate box(es) below.
